COLLEGE FEE WAIVER REQUEST FORM

All students requesting fee waivers for college applications must pass in this form to the Guidance office. We require three days notice for each request. You must fill in dates that you received and took the SAT test. A college application waiver cannot be granted unless you have taken the SAT with a fee waiver. You are allowed up to four college fee application waivers. If you received a SAT fee waiver from another organization like Upward Bound or TRIO, you must receive the college fee waiver from that organization.
